The City of Menlo Park only accepts on-line digital Building permit applications with digital plans and supporting documents. All applications, plans and supporting documents are to be submitted digitally through the City’s Accela permit management system using the Accela Citizen Access Portal (ACA).

Each form listed includes a link to more detailed information available on the Building Division's web site regarding what the form addresses and when it is typically required..
Building Permit Application and Data Sheet Forms
A completed Building permit application is required with all permit submittals. A Data Sheet for Building Permit Application is not required for all permit applications. When required, the information can be included in the plan set rather than submitting a separate form. Engineering Division Submittal Checklist
A completed Engineering Division Submittal Checklist is required as part of a complete Building permit application for certain projects. Please see One- and Two-Family Homes and ADU Permit Information and Nonresidential and Multi-Family Permit Information for when this checklist is required.
Electrical Load Calculations
A completed Electrical Load Calculation form is required as part of a complete Building permit application for proposed new, additions and alterations to single-family home construction projects and photovoltaic projects. Deferred Submittal Form
A completed Deferred Submittal form is required to be submitted as part of a complete Building permit application whenever deferred submittals are being requested. Please see Submitting a Deferred Submittal for when this form is required.
Application for the Use of Alternate Means and Methods of Construction
Request Form
A completed Application and Conditions of Approval for Alternate Methods of Construction request form is required to be submitted as part of a complete Building permit application whenever an alternate means and methods are being requested. please see Alternative Materials and Methods of Construction Application for when this form is required.
Special Inspection Form
A completed Special Inspection Form is required to be submitted as part of a complete Building permit application when the structural design requires special inspection per Section 1705 of the 2025 CBC. Please see Special Inspection
AC to HP Unreasonable Hardship Form
A completed AC to HP Unreasonable Hardship form is required to be submitted as part of a complete Building permit application when the cost of compliance creates an unreasonable hardship. Please see CALGreen: Replacing A/C Condenser with Heat Pump Req. for more information that clarifies when a project is eligible to request an unreasonable hardship.
Disabled Access Unreasonable Hardship Form
A completed Disabled Access Unreasonable hardship form is required to be submitted as part of a complete Building permit application when the cost of compliance creates an unreasonable hardship.
